Carers Support

Are You A Carer?

We hold a register of patients who are carers; if you would like to be added to this, please ask at reception. Being on the register ensures that you benefit from annual flu vaccinations and if you wish, the details can be passed on to the Carers Association who may be able to offer extra support and information if required.

Patients who have a Carer are requested to complete a Carer's Registration Form so we have their permission to share details of their medical care with those looking after them.
